Referring a worker
Complete the referral form and provide a copy of all relevant documents to the tribunal—use our tribunal referral checklist to make sure you include all relevant information about previous claims so that:
- the tribunal is aware of previous similar injuries
- assessment can be apportioned accordingly.
The referral is an official register of documents being submited to a tribunal. If the claim has been to a previous tribunal, you only need to list and attach documents received since that tribunal. Do not forward the referral if there are reports outstanding.
You don't need to include:
- accounts
- claim payments
- travel itineraries
- internal emails or memos
- general correspondence from insurer to worker or doctors
- witness statements or investigation reports.
We've put together a list of injuries and illnesses by tribunal to help identify the appropriate tribunal for your referral.
